Claim Form

All claims for personal injury, property damage, or other losses against the City of Rancho Cordova must be submitted in accordance with the California Government Code Section § 810 et seq.). To file a claim, please complete a Claim Form and submit it to the City Clerk’s Department. Be sure to include any supporting documentation that may assist in the review of your claim, such as:

  • Repair estimates or invoices
  • Photographs
  • Proof of ownership
  • Any other relevant information

The Claim Form must be signed and dated with an original signature. Claimants are encouraged to retain copies of all materials submitted for their records.

What Happens Next

Once received, claims are reviewed by the City Clerk’s Department and forwarded to the City’s Risk Manager and third-party insurance administrator for evaluation. Written notification will be provided to the claimant regarding the status or disposition of the claim.
